Cefalosporine e anemia emolitica

Riportiamo il numero totale di segnalazioni di anemia emolitica da cefalosporine presenti nel database:
L’anemia emolitica da farmaci è una reazione avversa immuno-mediata o geneticamente determinata (ad esempio nei pazienti con carenza di G6PD), non comune ma spesso grave, che può determinare insufficienza renale, coagulazione intravasale disseminata e morte. Le cefalosporine, in particolare di seconda e terza generazione, sono annoverate tra le classi di farmaci potenzialmente responsabili di tale tossicità (1,2).
Nella banca dati del GIF sono presenti segnalazioni di sospetta anemia emolitica da cefalosporine, da cefotetan e da cefepime (vedi la tabella sopra). Inoltre, mentre la scheda tecnica dei medicinali a base di cefotetan prevede tale possibilità (positività al test di Coombs), quella dei medicinali a base di cefepime non menziona tale rischio.
Nonostante in letteratura siano presenti diversi case-report e case-series riguardanti tale reazione avversa (in particolare a carico di cefotetan e, più in generale, per le cefalosporine di II e III generazione), non esistono delle precise stime di incidenza. Una possibile raccomandazione può essere quella di Stroncek (1), secondo cui “non è facile distinguere una emolisi iatrogena da altre cause di anemia emolitica, ad esempio quella di origine trasfusionale. Se l’anemia emolitica dovesse manifestarsi sarebbe necessario eseguire un’anamnesi farmacologica completa del paziente oltre che effettuare immediatamente un test per la ricerca di anticorpi farmaco-dipendenti. Inoltre, considerando che in questi pazienti vi può essere reattività crociata tra gli anticorpi indotti dal cefotetan con la cefalessina, in futuro bisognerà evitare l’esposizione allo stesso cefotetan e suoi analoghi e, più in generale, alle cefalosporine”.
